Circle of Care Hospice Care services are for individuals facing a terminal medical condition, illness or disease wanting to avoid hospitalization or forego curative treatments. Generally, hospice patients prefer to spend their final days in the comfort of their own home and focus on things that matter most to them and their loved ones.
Hospice Care is typically funded by Medicare Part A, Medicaid, MediCal, the Department of Veteran’s Affairs for qualifying veterans and their spouses, and most private insurance companies. Private insurance companies may have co-pays depending on your plan.
Circle of Care aims to provide compassionate comfort care, relief from pain and suffering, provide physical, mental, spiritual support to individuals in the final stages of life.
Yes. Patients with terminal prognosis needs to be certified by a physician.
Circle of Care provides an interdisciplinary approach that includes compassionate comfort care to support ones mental, physical, emotional and spiritual well-being, and symptom management that reduces suffering including pain management and relief from side effects associated with disease or illness. Circle of care also provides home care services that sustain activities of daily living, or ADL's, and Instrumental activities of daily living, or IADL's, and respite family care.
Hospice care is typically for patients in the last 6 months of life. However, if a patient's condition improves, Circle of Care hospice team will work to change to another care option.
