Circle of Care Home Health services are available for all individuals. Persons who may be recovering from an injury from an accident, or post surgery such as joint replacement or heart surgery, may benefit from short-term/episodic home health care. Persons diagnosed with a mental condition such as dementia or Alzheimer’s disease, or a chronic conditions like diabetes, COPD, cancer or diabetes, could benefit from long-term home health care.
Medicare, Medicaid, and many commercial insurance plans. The health care part of it is either paid by Medicare or private health insurance and the home care part is covered by LTC Insurance or private pay. When circumstances require an individual or family to self-pay, Circle of Care offers flexibility home health options. Yes, a physician must order Home Health care. The physician also determines what health care disciplines are necessary to meet the needs of each individual.
Home Health services are ordered by a physician and can include a variety of, physical, occupational therapy, speech therapy, medical social services, home health aide or registered dietician services. Circle of Care uses a multidisciplinary care approach that ensures medication prescriptions, and medical equipment are coordinated and fulfilled according the the physicians orders.
Typically members (RN, PT, OT, ST, SW, HHA) of the Inter-Disciplinary Team (IDT) visits 1 to 3 times per week at 1-hour per visit. To achieve speedy recovery, most patients under Home Health Care also hires private caregivers to assist them with daily activities if they have no family member or loved ones who can provide that kind of assistance.
Service can last as long as the client has the potential for improvement with home health care treatment. Coordination with the physician as well as insurance eligibility or self-pay requirements are needed.
